The facility must submit two different examination types for each module/sub module, per the following table. Each exam must be from a different patient (i.e. if Mr. Jones is submitted for a WB Bone exam, you must submit a different patient for the Bone SPECT exam). Whenever possible, submit normal nuclear medicine exams; however, abnormal exams may be submitted if no high quality normal studies are available.

Required Nuclear Medicine Exams for Module 1, Module 2 and Module 3
Module 1: Planar
Module 2: SPECT
Module 3: Nuclear Cardiology
Sites are required to submit two separate planar studies for each unit to be accredited. Whole body or spot bone is required, plus one exam from the following list (if unit does not perform bone scans, two exams must be selected from the list below; if MUGA performed on unit and site not applying for Nuclear Cardiology module, you MUST select MUGA as one of the exams):
For each scan submitted, the site is required to submit the full set of images
* MUGA: Images of the entire cardiac cycle for each projection must be submitted for the MUGA, as well as an image of the ejection fraction curve. Images submitted must match the submitted written procedure. i.e., if the written procedure states 3 views are acquired routinely, then a splash page for all 3 views must be included with the curve.
**Breast: Breast studies will fail if they are not correctly labeled (RDD, LCC, RMLO, LMLO). Breast may only be selected for planar imaging for dedicated breast scanners. General nuclear medicine scanners cannot select the breast exam.
Sites are required to submit two separate SPECT studies for each unit to be assessed. Bone SPECT is required, plus one exam from the following list (if unit does not perform bone scans, two exams must be selected from the list below; only one myocardial perfusion scan may be selected, and if selected, site must also apply for the Nuclear Cardiology module):
Images must be displayed in multiple planes including transverse, coronal and sagittal.
* If submitting a DaTscan for Brain SPECT, images MUST include the axial DICOM slices and must display all 3 planes.
For all other SPECT submissions, Axial only DICOM slices are strongly recommended IF they display in 3 planes.

SPECT Myocardial perfusion is required, plus one of the following:
Two different patient exams are required for this module even if the myocardial perfusion exam is submitted under the SPECT module.
SPECT myocardial perfusion images must include the stress and rest slices (SA slices must include apex to base, VLA and HLA slices must show LV slices "wall to wall" - Two pages of slices may be necessary to show the entire LV apex to base and wall to wall), as well as an image of the time activity or volume curve depicting the ejection fraction. In addition, the images depicting the ROIs (end-systole and end-diastole) used to generate the EF must be submitted.
* MUGA: Images of the entire cardiac cycle for each projection must be submitted for the MUGA, as well as an image of the ejection fraction curve.)
