Options Following a Failure or Deficiency: Radiation Oncology (Revised 12-12-19)

Revision History

Radiation oncology facilities not granted accreditation may be either deferred or denied. In either case, the facility has 90 days to submit a Corrective Action Plan to the ACR. After the Corrective Action Plan is approved by the Committee on Radiation Oncology Practice Accreditation, the facility may proceed as follows:

  • Facilities whose accreditation was deferred: The facility may be required to perform a self-audit (measures for self-audit will be selected by the Committee) and submit the results no later than 6 months after receipt of a response to the corrective action. Following Committee approval of the self-audit, the facility may be granted a three-year accreditation. The Committee may request a scheduled on-site survey (SOSS) if the Corrective Action Plan is approved. Additional fees may be applied, such as On-Site Surveyors’ expenses, including travel and lodging.

  • Facilities whose accreditation was denied: The facility will be required to participate in a follow-up survey (6-9 months after receipt of a response to the Corrective Action Plan). A re-application fee of $5,000 must be submitted with the survey agreement. Additional fees may be applied, such as On-Site Surveyors’ expenses, including travel and lodging. The surveyors will complete a report of their findings, which will be reviewed by the Committee. Following Committee approval of this report, the facility may be granted a three-year accreditation.

An appeal process is available to a radiation oncology facility that disagrees with the accreditation report. To appeal, the chief of radiation oncology submits a written request to the Chair of the Committee within thirty days of receipt of the accreditation report.
